Adopt A Patient Program Application

Providing financial relief and hope to cancer patients and families when they need it most.

At Jamie’s Hope, we know that cancer affects far more than a person’s health. Between treatments, time away from work, travel to appointments, medical expenses, and the everyday bills that don’t stop when cancer begins, the financial burden can quickly become overwhelming.

The Adopt-a-Patient Program was created to provide a little breathing room during an incredibly difficult time. Each month, Jamie’s Hope selects one cancer patient to receive $1,000 in financial assistance to help with the needs that matter most to them and their family—whether that means groceries, housing, utilities, transportation, medical expenses, or simply keeping up with everyday life.

More than financial assistance, this program is our way of reminding patients and families that they are not alone in this fight. By sharing the stories of our Adopt-a-Patient recipients, we hope to surround each family with support while helping others better understand the very real impact cancer can have on every part of life.

How it Works:

1. Submit your application by completing the form and telling us about your cancer journey, current circumstances, how financial assistance would help, and some recent photos.

2. Our team reviews each application to determine eligibility and financial need.

3. One recipient is selected each month to receive $1,000 in financial assistance.

4. If selected, we’ll reach out directly to complete any necessary documentation, learn more about your story, and coordinate the best way to provide your financial assistance.

Eligibility

• Applicants must be currently diagnosed with cancer or receiving cancer-related treatment or care.

• Applicants must reside in the United States.

• Applications may be submitted by the patient or by a parent, legal guardian, or authorized caregiver on their behalf.

• Applicants must demonstrate a financial need related to or impacted by their cancer journey.

• Applicants must explain how the $1,000 financial assistance award would help them or their family.

• Previous Jamie’s Hope Adopt-a-Patient recipients are not eligible to receive assistance a second time.

• Selected recipients must be willing to allow Jamie’s Hope to share their name, story, and photographs for marketing, fundraising, and awareness purposes and complete the required media release.

• Jamie’s Hope may request additional documentation to verify eligibility or information provided in the application.

• Submission of an application does not guarantee selection or financial assistance.
